Billing Details
I understand that there are no refunds or reductions offered for missed classes or if my student is unable to complete the camp. I also understand that an 80% refund is offered if for any reason my child is unable to attend the camp and a notice of 5 days is given before the first day of camp.*
I hereby release Center Stage Academy of the Arts, LLC, and all staff members from any and all claims of damages or injury suffered by the above registered student(s) in connection with or by their associations with the Center Stage Academy of the Arts, LLC. This includes my heirs who may not act in my behalf. (Note: Student's should be covered under an individual insurance policy as Center Stage Academy of the Arts will not be held responsible for these claims.)*
I acknowledge that the Center Stage Academy of the Arts has the absolute right and permission to use audio, images, or video of my child obtained in rehearsals or performances in its promotional materials and publicity efforts. I understand that this media may be used in publication, print ad, direct-mail piece, electronic media, or other promotion. I release Center Stage Academy of the Arts, their photographer, employees, agents, and designees from any liability for any violation of any personal or proprietary right I may have in connection with such use.*
I acknowledge and accept full responsibility and guarantee payment for any charges accrued on my account while my student(s) are enrolled in the Center Stage Academy of the Arts.*
I understand that the Center Stage Academy of the Arts agrees that the credit/debit card provided with this registration will not be used for any other reasons, other than those specified within this form unless prior written authorization is received.*

Parent Code of Conduct
Casting for musical theater is an involved process. By signing this contract I understand that every child registered in Center Stage Academy is accepted into a program, however casting of particular characters or types of roles is determined through an audition process. I will do my best to set the proper expectation with my student and help them to see the exciting challenges and opportunities found in any any assigned role.*
Should I need to personally address any concerns I have regarding the musical theater program, I will contact Center Stage or Miss Katherine directly and request a time be scheduled to meet outside of the classroom. I agree to address my concerns in a patient, respectful manner and never in front of students.*
I understand that reading Center Stage remind texts, emails, and handouts in their entirety is required.*
